No Surprises Act disputes turn on records the federal IDR process creates: the certified IDR entity's determination, the selected offer, the initial payment, patient cost sharing and the date the plan paid. Under 45 CFR 149.510, the plan owes the selected offer, less the initial payment and cost sharing, within 30 calendar days of the determination.
